Acknowledgment of Risk

I understand that martial arts training, Brazilian Jiu- Jitsu, MMA, fitness training, strength training, conditioning, and related activities involve inherent risks of injury.

These risks may include, but are not limited to:

Cuts, bruises, sprains, and strains

Joint injuries or broken bones

Concussions or head injuries

Serious injury or permanent disability

I voluntarily choose to participate in activities at TLNT MMA & Fitness , understanding that participation is at my own risk.

Assumption of Risk

I knowingly and voluntarily assume full responsibility for any injuries or damages that may occur during participation in any activity at TLNT MMA & Fitness.

I understand that martial arts training may involve physical contact with instructors or other participants.

Light Sparring Acknowledgment

I understand that certain classes may include light controlled sparring or grappling drills as part of normal martial arts training. I agree to participate responsibly and follow all safety instructions from instructors.

Release of Liability

To the fullest extent permitted by law, I release and hold harmless TLNT LLC, TLNT MMA & Fitness , its owners, instructors, employees, volunteers, and affiliates from any claims, injuries, damages, or losses arising from participation in any class, training session, or event.

Medical Responsibility

I confirm that I am physically capable of participating in martial arts and fitness activities. I understand that TLNT MMA & Fitness does not provide medical insurance for participants.

In case of emergency, I authorize staff to seek medical care if necessary.

Media Release

I grant TLNT LLC / TLNT MMA & Fitness permission to use photographs or video taken during classes or events for marketing, promotional, and social media purposes without compensation.

Minor Participants

If the participant is under 18, the parent or legal guardian signing this agreement accepts full responsibility for the minor’s participation and agrees to all terms of this waiver.
